Special Dietary Needs & Allergies

You may complete a Special Feeding Form for your child and send to your campus nurse.

If your child has special dietary needs, you will need to complete a Special Dietary Needs Form that must be signed by a medical professional. This will assist with any food allergies, diabetic restrictions or special textured food preparations (grinding, pureeing, etc). This form should be completed in detail by the medical professional. It is recommended that a new form be submitted each year, preferably before the start of the school year.

If you have any questions or concerns, please contact Kerri Sanford, Child Nutrition Supervisor, at (936) 829-6262 or by email at ksanford@dibollisd.org.
